why do projects like spaceref.com/news/viewpr.html?pid=5413 or insignificant ones like emsgamers.com/ fail? “For example, NASA did not complete a project plan or develop sufficient information.” here’s a brief list:

1 bad leadership/poor communication (the first one is nothing new)

2 no project plan, no idea what the project really is

3 vague requirements

4 inefficiency and wasting everyone’s time

i wouldn’t really recommened buying one of the thousands of “leadership” books on amazon, but for some people, it could help.
